Patients frequently display normal cholesterol mass alongside dangerous particle congestion. Most labs rely on the outdated Friedewald formula to estimate these figures, masking severe heart disease risk in millions of patients.
Otvos challenged the widespread belief that large, buoyant LDL particles are inherently safe. When total particle counts escalate, individual particle size offers no protection. Patients with Familial Hypercholesterolemia carry large particles but suffer early mortality because sheer volume overwhelms arterial defenses. Small particles appear dangerous primarily because they signal a higher overall particle count.
Particle volume drives the damage, regardless of size.
Nuclear magnetic resonance spectroscopy also reveals early metabolic breakdown. Standard medicine waits for fasting blood sugar to breach high thresholds before diagnosing diabetes. By that point, patients have often lost half of their pancreatic beta cell function. Otvos highlighted how the NMR-derived LPIR score flags insulin resistance years earlier, opening a window for primordial prevention.
Early detection also exposes the limitations of standard treatments. While lifestyle interventions move the LPIR score significantly, common medications like metformin often fail to repair the underlying cellular resistance.
Waiting for blood sugar spikes or cholesterol weight spikes means treating disease after irreversible damage has occurred.
